On May 5, 2023, the World Health Organization declared that the COVID-19 pandemic no longer constituted an international public health emergency.

However, this does not mean the complete disappearance of the threat posed by COVID-19. Instead, the focus of society has shifted from the pathological changes during the acute phase of infection to the “Long COVID Syndrome” that occurs after recovery.

There are currently no specific guidelines or consensus regarding the treatment and clinical management of Long COVID.

Overall, the treatment goal is patient-centered, aiming to optimize specific organ dysfunctions and improve the quality of life for patients.

 

Recently, a study titled “Effects of six-month creatine supplementation on patient- and clinician-reported outcomes, and tissue creatine levels in patients with post-COVID-19 fatigue syndrome” was published in Food Science & Nutrition.

The results of this clinical trial indicate that dietary supplementation with creatine can significantly improve the health of Long COVID patients, with notable improvements in overall fatigue, body pain, and reduced concentration, among other related symptoms.

About Creatine:

Creatine is primarily synthesized in the liver, kidneys, and insulin-producing cells in the pancreas. It was first discovered in skeletal muscle by a French scientist in 1832. Despite its amino acid-like structure, creatine does not participate in protein synthesis. Its primary role is to promote and maintain muscle growth, aid in ATP synthesis, and effectively delay muscle fatigue.

Although creatine serves various vital physiological functions in the human body, under normal circumstances, the body can produce only about 1 gram of creatine per day. Therefore, for individuals engaged in frequent high-intensity exercise, the naturally produced creatine is often insufficient, leading people to use creatine supplements to enhance performance and increase muscle mass.

 

 

Creatine’s Improvement of Long COVID:

This clinical trial included 12 Long COVID patients between the ages of 18-65 who had tested positive for COVID-19 within the past 3 months.

The trial lasted for 6 months, and the 12 participants were randomly assigned to either a placebo group or a group taking 4 grams of creatine monohydrate daily.

The primary and secondary outcome measures of the study included 5 dimensions: fatigue, tissue creatine levels, patient-reported outcomes, clinical reports of exercise tolerance, as well as the incidence and severity of side effects.

The results from the 3-month and 6-month follow-ups showed that creatine intake led to a significant increase in tissue creatine levels in the patients’ inner thigh muscles and right frontal white matter compared to baseline (p<0.05). In contrast, the placebo group did not exhibit any changes in tissue creatine values throughout the trial. Two-way variance analysis revealed a significant difference in tissue creatine levels between the placebo and creatine intake groups (p<0.05), with the creatine intake group showing greater improvements in creatine levels in the inner thigh muscles, left frontal white matter, and right frontal white matter.

With the increase in intramuscular creatine levels, the creatine intake group also experienced a significant improvement in overall fatigue symptoms compared to the placebo group. Symptoms related to fatigue syndrome, such as loss of taste, difficulty breathing, body pain, headaches, and difficulty concentrating, all showed marked improvement.

The results of adverse event monitoring showed that apart from one patient experiencing transient mild nausea after taking creatine, there were no other reported adverse reactions in the creatine intake group. Therefore, this small-scale trial with 12 participants concludes that creatine intake by Long COVID patients has virtually no adverse reactions or side effects.

In summary, this is the first study to assess the effectiveness and safety of creatine supplementation on the prognosis of fatigue, tissue bioenergetics, and patient-reported outcomes in Long COVID patients. Experimental data indicates that creatine can increase creatine levels in the brain and skeletal muscles, improving symptoms associated with post-COVID-19 fatigue syndrome. However, further large-scale trials are needed to definitively confirm the effectiveness of creatine supplements in alleviating Long COVID symptoms.
