The risk of spontaneous abortion after receiving COVID-19 vaccine during pregnancy

The risk of spontaneous abortion after receiving COVID-19 vaccine during pregnancy

JAMA+NEJM: The risk of spontaneous abortion after receiving COVID-19 vaccine during pregnancy.

Infection of the new coronavirus during pregnancy may be related to severe maternal morbidity. In the United States, two mRNA COVID-19 vaccines have been approved for pregnant women, but relevant safety data are limited.

Moreover, the data on the safety of the COVID-19 vaccine for pregnant women mainly comes from passive monitoring, and the study lacks an unvaccinated control group.

Spontaneous abortion is a priority result of research on the safety of vaccination for pregnant women, and concerns about the risk of spontaneous abortion are also the main obstacles to vaccination during pregnancy.

September 2021, ” American Medical Association Journal ” (JAMA) published a report entitled from Yale University and other units: Spontaneous Abortion Following COVID-19 Vaccination During Pregnancy papers.

This paper introduces the results of case-control monitoring of COVID-19 vaccination during pregnancy and spontaneous abortion.

Data analysis of this large-scale study shows that compared with unvaccinated pregnant women, the two new mRNA vaccines will not increase the risk of spontaneous abortion in pregnant women.

The risk of spontaneous abortion after receiving COVID-19 vaccine during pregnancy

The data for this study comes from the US Vaccine Safety Data Link, a collaborative project between the US Centers for Disease Control and Prevention and 9 health systems, covering 3% of the US population.

The research team used a proven pregnancy algorithm that combines diagnostic and program codes and electronic health record data to identify the gestational age of spontaneous abortion and continued pregnancy.

Based on these data, the research team analyzed and compared the probability of spontaneous abortion or continued pregnancy within 28 days after vaccination.

Among 105,446 pregnant women who received the COVID-19 vaccine, there were 13,160 spontaneous abortions and 92,286 persistent pregnancies.

Overall, 7.8% of these pregnant women received at least one dose of Pfizer-BioNTech mRNA vaccine; 6.0% received at least one dose of Moderna mRNA vaccine; 0.5% received Johnson & Johnson’s adenovirus vector vaccine. The rest were not vaccinated against the COVID-19 vaccine.

8.0% of persistent pregnancies and 8.6% of spontaneous abortion pregnant women received the COVID-19 vaccine within 28 days.

Compared with the persistent pregnancy group, the probability of spontaneous abortion pregnant women receiving mRNA vaccine within 28 days before miscarriage did not increase, and between the two mRNA vaccines There is no difference.

Because the number of pregnant women vaccinated with adenovirus vector vaccines is too small, it is impossible to evaluate its safety to pregnant women.

The risk of spontaneous abortion after receiving COVID-19 vaccine during pregnancy

October 2021, ” New England Journal of Medicine ” (NEJM) published an article from the US Centers for Disease Control and Prevention, the article reported that the risk of pregnant women, COVID-19 after inoculation mRNA vaccine spontaneous abortion.

A total of 2456 participants who participated in the CDC v-safe Covid-19 pregnancy registration met the inclusion criteria of this study. 2022 participants reported continued pregnancy at 20 weeks of pregnancy, and 165 participants had spontaneous abortions.

After standardizing the age of pregnant women, the cumulative risk of spontaneous abortion from 6 weeks to less than 20 weeks of pregnancy for these pregnant women who received the mRNA COVID-19 vaccine was 12.8%.

Compared with the two historical cohorts representing the upper and lower limits of spontaneous abortion risk, the above-mentioned risk Within the expected range.

Therefore, these data show that the risk of spontaneous abortion after vaccinating the mRNA COVID-19 vaccine before conception or during pregnancy is consistent with the expected risk of spontaneous abortion without vaccination .

In addition, in June 2021, ” New England Journal of Medicine ” (NEJM) published a research paper US Centers for Disease Control and Prevention, the paper crown on the new mRNA vaccine side effects in pregnant women conducted a comprehensive analysis.

Statistics show that compared with non-pregnant women, pregnant women are more likely to have pain at the injection site after receiving the mRNA vaccine, but have a lower probability of headache, muscle pain, chills, and fever .

The study found that compared with non-pregnant women, the frequency of injection site pain is higher in pregnant women, while the frequency of headache, myalgia, chills and fever is lower.

Among the 3958 participants who participated in the v-safe pregnancy registration, 827 were pregnant, of which 115 (13.9%) had miscarriages.

Adverse neonatal outcomes included premature delivery (9.4%) and small gestational age (3.2%). These data show that pregnant women vaccinated with the mRNA COVID-19 vaccine did not show obvious safety issues .

These studies provide more and more evidence for the safety of the COVID-19 vaccine (mRNA vaccine) during pregnancy .
