Myocarditis / pericarditis in which the heart muscle (myocardium) and the surrounding membrane become inflamed. Cases that develop after vaccination with the new coronavirus have been reported very rarely in Japan and overseas, especially among young men.

The third inoculation is expected to begin in earnest at the beginning of the year. In most cases, the symptoms are mild and heal spontaneously, but I would like to deepen my understanding of the symptoms and lead to early detection in the unlikely event.

Japan: Myocarditis/pericarditis may occur in young men after COVID-19 vaccination.

Most of the causes of myocarditis and pericarditis are viral infections such as colds and flu. Inflammation of the heart muscle and pericardium causes chest pain, dullness, shortness of breath, and abnormal pulse several days after cold symptoms such as fever, chills, and diarrhea.

Another feature is that chest pain changes depending on the position, such as increasing pain when lying down and relieving when you wake up and lean forward. According to the national total, the number of patients in FY2009 was about 15,000.

To confirm the diagnosis, it is necessary to evaluate the degree of inflammation by electrocardiogram, ultrasonography, blood test, magnetic resonance imaging (MRI) test, etc. In most cases, it will heal spontaneously in a few days, so there are many cases where you do not see a doctor or you do not need treatment when you see a doctor.

However, when it becomes severe and the heart cannot contract well, blood cannot be sent to the whole body and it is fatal. There is no choice but to continue symptomatic treatment until recovery. If it becomes chronic, it may lead to intractable disease such as dilated cardiomyopathy.

Cases of suspected onset after inoculation have been reported by Moderna and Pfizer, which inject part of the genetic information of the virus, “messenger RNA (mRNA),” to cause an immune response in the body. According to the manufacturer’s report, the most frequent per million people are men aged 12 to 19 years old who hit Moderna, 28,83 people = table. For young men, there are many reports after the second inoculation for both Moderna and Pfizer.

In mid-October, a man in his thirties examined by Yoshihiro Seo (54), a professor of cardiovascular medicine at Nagoya City University School of Medicine, is “a case suspected of being related to a vaccine.”

An abnormality was found in the electrocardiogram during a regular medical examination two weeks ago, and a detailed examination was performed, but it has already improved.

He had no history of illness, and when he was interviewed, he was examined three days after vaccination. “It is undeniable that myocarditis and pericarditis may have healed without symptoms,” he said.

The causal relationship between the vaccine and myocarditis / pericarditis is unknown, but Mr. Seo, who is also a member of the COVID-19 countermeasures mission team of the Japanese Circulation Society, says, “It may be caused by an immune response.” The analysis is also consistent with the tendency of young people to have a second, rather than first, side reaction to the vaccine that occurs when immunizing the virus. Estradiol, a female hormone with anti-inflammatory properties, may have an effect on young women.

However, myocarditis and pericarditis occur even when infected with COVID-19, and the frequency is significantly higher than after inoculation. A

ccording to the Ministry of Health, Labor and Welfare, among the patients hospitalized by May, 23 males and 10 females were suspected of having myocarditis / pericarditis, and 148 and 600 per million, respectively. Seven people.

For both men and women, there are overwhelmingly more than 600 people after inoculation made by Moderna and 2 people made by Pfizer. That is why the Ministry of Health, Labor and Welfare’s expert group states that “the benefits of vaccination outweigh the risks.”

On the other hand, from October, the country says that men in their teens and twenties can choose Pfizer products even if they are pre-ordering Moderna products or have already hit the first time.

The third vaccination will begin in December for healthcare workers. It is expected to start sequentially from the beginning of the year for the elderly and under 65 years old. Mr. Seo says, “If you feel any discomfort or suffocation in your chest after vaccination, see a medical institution where you can get an electrocardiogram.”
