The George Institute for Global Health has released data from the Phase III INTERACT3 study.

Innovative therapy for stroke caused by intracerebral hemorrhage (ICH) significantly improves survival and minimizes severe disability, the study confirms.

The findings were presented at the European Stroke Organization meeting in Munich, Germany, and published in The Lancet.

 

The INTERACT3 trial is the first randomized controlled study to provide clearly positive results for the treatment of ICH. The implementation of this new treatment program, known as “bundling of care,” focused primarily on rapid control of high blood pressure has resulted in good recovery, reduced mortality, and an overall improved life for patients with this serious disease. quality.

ICH, often called hemorrhagic stroke or intracerebral hemorrhage, is the second most common type of stroke and the deadliest, with 40% to 50% of patients dying within 30 days. The condition, which occurs when blood leaks from blood vessels into brain tissue, accounts for more than a quarter of all strokes, affecting about 3.4 million people each year.

In the INTERACT3 study, 144 hospitals in 10 countries (9 middle-income countries and 1 high-income country) enrolled more than 7000 patients.

 

The research team evaluated the effectiveness of a new combination of care that included early intensive systolic blood pressure lowering, tight blood sugar control, fever management, and rapid reversal of abnormal anticoagulation.

They found that using this new treatment regimen reduced the likelihood of adverse functional outcomes, including death, after six months, compared with usual care. It was estimated that 1 additional death could be averted for every 35 treated patients.

 

Central to this was a rapid reduction in systolic blood pressure, reaching target levels within an average of 2.3 hours (range, 0.8 to 8.0 hours) compared to 4.0 hours (range, 1.9 to 16.0 hours) for the control group.

The interventional regimen resulted in statistically significant reductions in mortality, number of serious adverse events, and length of hospital stay, and demonstrated improvements in health-related quality of life.

 

The burden of intangible cultural heritage is highest in low- and middle-income countries. In 2019, 30% of all strokes in low- and middle-income countries were intracerebral hemorrhages, almost double the proportion in high-income countries (16%).

This is partly due to the high prevalence of hypertension and limited resources for primary prevention strategies, including identification and management of stroke risk factors by health services.

 

Co-lead author Dr Song Lili, Head of the Stroke Program at The George Institute for China Research, said: “The lack of effective treatments for intracerebral hemorrhage has created a pessimistic perception that nothing can be done for these patients.

How existing treatments can be used to improve outcomes in resource-limited settings. We hope this evidence will inform clinical practice guidelines worldwide and help save many lives.”
